项目摘要

本研究の前半に行った介護付き有料老人ホームにおける施設内看取りやACP（事前意思確認）の実施状況に関する実態調査の結果について、22nd World Congress of Gerontology and Geriatrics(IAGG 2022）にてポスター発表を行った。また、本調査の結果については、まだ論文投稿に至っていないため、2023年度は「日本エンドオブライフケア学会」への投稿を視野に執筆を進めている。本研究の後半に実施を予定していた、介護付き有料老人ホームに勤務する看護師にや施設長、ケアマネージャー、介護職等を対象とする「施設内での看取りやACPについて」のインタビュー調査に関しては、COVID-19の影響にて施設内の立ち入りが制限されたり、施設スタッフの業務が煩雑になる状況が続いていたことから、2022年度もインタビューの依頼が困難な状況が続いた。しかし、年度後半になって、ようやく感染状況にも回復の兆しが見えてきたことから、現在、インタビュー対象者のリクルートに入り、インタビュー実施に向けての準備を進めている。ACPの重要性は、ここ数年で少しずつ浸透しつつあるが、先の死を見据えて「日常生活」の中でのACPの実践率は低い。本研究の実態調査でも、日常でのACP実施は半数に満たなかった。介護付き有料老人ホーム入居者は、特別養護老人ホームに比べると日常（安定期）の期間が長く、かつ自立度も高いため高齢者本人への意思確認が取りやすい施設と言える。看護師を含めスタッフへのインタビューを通して、介護付き有料老人ホームにおける中長期的なACP支援のあり方を検討する。
