前年度までの研究に加え、下記内容を追加して研究した。【目的】地域医療や地域包括ケアシステムを理解するために地域基盤型教育(CBME)が重要とされている。CBMEにおいて指導医は重要な存在であるが、現状では彼らの意見が教育に反映されているとは言い難い。地域医療を担う指導医が考える、医師に求める資質・能力及びそれらを獲得・涵養するために実施したい教育、教育活動への影響因子を明らかにすることを目的とする。【方法】福島県内の地域医療実習を担当する指導医13名を対象に半構造化インタビューを実施し、逐語録を主題分析法により分析した。【結果】指導医は、地域医療を担う医師に必要な資質・能力として「コミュニケーション能力」「患者中心の医療・視点」「地域愛・人間愛」等を重視していた。それらを獲得・涵養するために実施したい教育として、地域住民を含めた地域全体と関係性を深められるような教育や、地域への愛着を育むような教育が挙がった。教育活動の促進因子として「教育することの喜び」等、阻害因子として「環境要因」「個人要因」等が抽出された。【考察】地域医療を担う指導医が特に重視している「コミュニケーション能力」「患者中心の医療・視点」の涵養のためには、地域との深い関わりを基盤とした教育が必要である。地域との関わりの重要性の認識は4 relationships model(Worley, 2002)における医療機関外の地域を学習の場とする「社会の軸」の視点に沿って描出することができた。特に低学年の医学生において生活、文化、社会的背景を含めた地域全体を俯瞰できるようなマクロな視点が得られる教育の開発の重要性を示唆し得た。教育に関して孤立しがちである地域の指導医と大学との連携を密にし、FDなどを通じて大学-地域間だけでなく各地域間の指導医の関係性も深める事が必要であると考えられた。
