项目摘要

本研究は、NICU入院中の低出生体重児の体動を長時間連続的に測定し、客観的に定量的に評価できる体動計測システムを開発すること、そして児の定量化された体動数とNICU退院後の児の成育・発達指標との関係を明らかにすることを目的とする。今年度は、本研究で開発した圧電センサを用いた小型の体動計測システムを用いて、機器の妥当性および信頼性の確認を検討しながら一部のプログラム改良を行いデータ蓄積ごとに検討した。対象児は施設の出入りの制限があるなかで、臨床の協力と児の観察とデータ計測が可能な研究分担者を加えて分析および検討を重ねた。その成果は、2022年12月の第42回日本看護科学学会学術集会で優秀演題口頭発表賞を受賞して、より建設的な助言や評価を得た。その研究テーマは、「圧電センサと画像解析を併用した新生児の自発運動の計量化と動きの関連」であり、新生児の自発運動による体動の圧電位と動画による部位（頭部、体幹、四肢）別の動きの特徴が示唆された。また同年11月に第6回日中韓看護学会にて、研究テーマ「Comparison of body movements between low-birth-weight infants and normal infantsｃ」を発表した。客観的に数量化した正常体重児と低出生体重児の違いについて、双方とも体重に相応した体動数が観察された。事例数の少なさから結論は保留だが2500ｇ以上と2500ｇ未満の児で体動の特性も異なる可能性が示唆された。これらの知見は体重の背景にある早産か正期産かの違いも考えて考察する。現時点の課題として、低出生体重児の追跡データの脱落率が約50％あるため、体動と児の発達と追跡データの分析を深めるためにも、さらに対象児のデータを増やす必要がある。
