项目摘要

本研究で取り組む2つの課題（①医療サービスのマクロスケール（全国的）空間構造の把握，②医療サービスのミクロスケール（地域）格差とその差違）のうち，前年度に引き続き，前者に関する分析に注力した．全国のメッシュ人口データ（三次メッシュデータ・国勢調査）と病院・診療所の位置データ（全国病院・診療所位置データベース(PARESA-Medical病院・診療所）を用いて，医療機関へのアクセシビリティに関する地域間格差分析を行った．北海道および北東北の各県において，病院・診療所（医療サービス）へのアクセスが明らかに劣る地域があることが認められる．それに対して，大都市圏においてはアクセシビリティ（物理的アクセシビリティ）の良さが際立ち，明らかに地域間格差がある．この結果の一部に関しては口頭発表を行った．引き続き，結果を論考としてまとめるべく分析を進めるとともに，構成（論文の章立て）等について検討中である．後者の視点も含めた分析として，北海道を事例に公立病院の経営状況に関して，その規定要因を，医療機関周辺人口との関係から把握することを目指して分析中ではある．本分析を深めるために出張も行い，そこで意見交換も行った．また，その際，youtube内番組の出演という形での一部成果発表も行った．とはいえ，後者の視点に関する分析については深まりを欠くと言わざるを得ない状況にある．両者の分析はいずれも地理学的には基本的な分析といえる作業である．それを論考としていかに説得的にまとめるか，どのような構成であれば分かりやすい・インパクトある研究成果として認められるか．代表者・研究協力者・研究分担者との討議・議論を通して，そうしたブラッシュアップを図ることが本研究の主眼である．それがコロナ禍の引き続きの影響によって充分に行えなかった．したがって研究実績としては不十分な点があることは否めない．

This study focuses on two topics: (1) understanding the spatial structure of medical services (nationwide),(2) differences in medical services (geographical), and (3) analysis of medical services (regional). National Population Survey (Third National Survey) and Location of Hospitals and Clinics (PARESA-Medical Hospitals and Clinics) were used to analyze regional differences in access to medical facilities. This analysis is based on the exchange of views. In the meantime, youtube's internal group played a part in the development of the results. The latter is related to the analysis.
