项目摘要

研究の目的は、健康寿命の継続を脅かすオーラルフレイルに陥りやすいとされる高齢者の口腔および嚥下機能と発声発語機能との関連性を調査し、嚥下反射惹起の指標であるサブスタンスP値を媒介として関連性を示すことである。更にその関連性を「能動的会話機会を提示するプログラムの開発」まで達成させていくことが最終目標である。オーラルフレイル予防・改善プログラムの開発を行うことによって、地域高齢者QOL拡大に貢献することを目指している。実施計画としては、１年目：実態調査と協力施設開拓、有効性の高い試料採取の方法の確立、２年目：会話機会と口腔機能の研究調査、3年目：上記調査継続とオーラルフレイル予防の能動的会話プログラム施行。4年目：研究のまとめ作業とした。行った内容として、初年度は、１）先行研究の検索作業、２）分担協力者との会議にて遂行手順を検討し、予備実験を開始した。1年目には、コロナ禍で実験対象である協力者の獲得に支障をきたしたため、情報収集と研究の妥当性と信頼性の確保にあてた。1～2年目の予備実験の内容は、本研究の軸となる唾液からのサブスタンスP値抽出のための条件を設定する実験であった。2年目である2021年は地域在住高齢者の実態調査を開始し、2022年度に終えることができた。口腔機能、摂食嚥下機能に加えて認知機能と発話機能のデータ収集を行い、日常の会話機会の自記式質問紙による情報回収を実施した。2022年3月末現在、約190名のデータを収集し統計分析を実施した。サブスタンスPと嚥下との関連が認められたほか、他の項目同士の重回帰分析およびロジスティック回帰分析においてオーラルフレイルに関与すると思われる嚥下機能低下に関わる項目を抽出した。今後は本来は3年目に実施する予定であった第２研究を実施していく予定である。
