手術後の生活機能を維持することは重要な課題であり、本研究では術後１年および５年の生活機能の評価を実施し、調整可能な関連因子を抽出する。関連因子に対し、術前からのプレハビリテーションとして介入できる方法について検討する。予定の非心臓血管手術を受けた患者で術後１年後の生活機能評価が可能であった2797例と術後１年までに死亡した124例の計2921例を対象に、術後１年の機能障害発生と関連する因子を検討した。結果、417例 (14.3%)が機能障害または死亡であった。年齢、BMI≧30、術前の脳血管障害、悪性疾患、拘束性肺障害、ステロイド内服、低アルブミン、低栄養が障害発生と関連する因子であった。この内、調節可能な因子は、栄養障害と肥満であり、術前からの栄養管理の重要性が示唆された。術後５年目の生活機能の評価は2021年４月より電話および郵送法により調査を開始した。術後5年後のフォローアップとして、2016年4月～2017年12月に手術を受けた患者2636名を対象に、1823名からデータ収集可能、107例で辞退、375例で死亡、331例で回答がなかった。術後5年後でも80％以上の患者で調査が実施できていた。肺外科手術を対象とした術前からの運動・栄養療法からなるプレリハビリテーションに関する前向き無作為比較試験を、2020年2月よりエントリーを開始した。新型コロナ感染症の拡大のため、外来受診の制限などにより、研究候補者233名のうち組み入れられたのは40名で、完遂者は15名であった。現在のプロトコールの問題点として、患者募集の方法や複数回の通院の困難性などが考えられ、患者や家族の負担を軽減できる遠隔のプレハビリテーションプログラムの構築が必要と考えられた。

The maintenance of life function after surgery is an important issue. In this study, the evaluation of life function at 1 year and 5 years after surgery was carried out, and the possible correlation factors were extracted. Correlative factors are discussed in the pre-operative approach. Among the patients undergoing non-cardiovascular surgery, 2797 patients were evaluated for possible life function at 1 year after surgery, 124 patients died at 1 year after surgery, and 2921 patients died at 1 year after surgery. Results: 417 cases (14.3%) died of functional impairment. Age, BMI ≥ 30, preoperative vascular impairment, sexual disorders, constraint lung impairment, oral administration, low blood pressure, and low blood pressure are associated factors. The importance of preoperative nutrition management is demonstrated by the internal and regulatory factors, nutritional disorders, and fertility. The evaluation of life function 5 years after operation began in April 2021. Five years after surgery, 2636 patients underwent surgery from April 2016 to December 2017, 1823 patients received surgery, 107 patients were dismissed, 375 patients died, and 331 patients answered. More than 80% of patients were investigated 5 years after surgery. Pulmonary surgery is a pre-operative exercise regimen that is not intended to be used as a comparative trial until February 2020. The number of new infections was 233, the number of foreign patients was 40, and the number of completed patients was 15.
