项目摘要

本研究の目的は、看護学習者の臨床判断の能力開発に活用できるようにラサター臨床判断ルーブリック(Lasater Clinical Judgment Rubric, 以下LCJR)日本語版の妥当性を検討し、臨床判断を拓く評価と臨床学習環境づくりに貢献できる教育指導者を支援するプログラムを開発することである。2022年度は、昨年度に実施した調査結果をもとに研究者間でプログラムの方針および内容を検討し、構成したプログラムを実施した。便宜的に抽出した近畿圏の医療機関に依頼し、1)各部署で看護学生や新人看護師の教育指導を行う看護職、2)看護師長以上の管理者ではないこと、3)看護学生及び新人看護師の教育指導の経験があることを条件とし、チラシのQRコードから申込フォームにアクセスする方法で教育指導者を募集した。28施設に所属する教育指導者71名から研究参加の同意を得た。研究参加者の希望日程と事前の質問紙調査の結果から背景が均質になるように介入群35名と対照群36名に割り付けた。両群ともにLCJR日本語版を用いた「臨床判断コンピテンシー開発セミナー」を実施し、LCJRの開発者(動画)及び日本語版作成者(対面)による講義を行った。介入群はセミナー終了後に「臨床学習環境づくりワークショップ」を実施し、看護学習者の臨床判断コンピテンシーを高めるために、施設の異なる教育指導者やファシリテーターとの共同作業を通して臨床学習環境について認識していることを分かち合い、省察することを目的としたワークショップを行った。ワークショップ後、介入群はLINEオープンチャットを開設し、2ヶ月間フォローアップをした。プログラムの終了後及び2ヶ月後に質問紙調査を実施した。プログラムの実施では介入群2名と対照群3名が不参加となり、事後の調査でそれぞれ1名が脱落した。今後、介入群32名と対照群32名を対象に分析を進める。

The purpose of this study is to examine the appropriateness of the Japanese version of Lasater Clinical Judgment Rubric (LCJR) for the development of clinical judgment abilities in nursing learners, to evaluate the contribution of clinical judgment to the development of clinical learning environments, and to support educators. After the intervention, the LINE was opened.
