项目摘要

オランダや北欧諸国などを中心に、訪問看護に関する文献を収集し、訪問看護師が直面する倫理的ジレンマの事例を検討したり、勉強会を行っている訪問看護ステーションの協力を得て、在宅ケア、訪問看護に固有の倫理問題の全体像について考察することができた。その結果、「病気を診て人を見ない」という病院医療のゆき過ぎたキュアを、ケアの視点でカバーすることが、施設内看護における看護倫理の中核をなしてきたが、在宅ケアでは「病気を診ること」も看護師の仕事になるということ、看護ケアの本質は、単なるケア、ケアリングではなく、医療とケアの媒介であり、それは在宅ケアを担う訪問看護師の役割のなかに明瞭に見て取れることを見出すことができた。さらに、医療技術とケアの媒介者としての訪問看護師は、近年のケア現場におけるICTやAIケアロボット機器の導入に際して、ハイテクノロジーとヒューマンタッチケアとの媒介を可能にし、技術とケアの両者をコーディネイトしていく役割を担う可能性があることを明らかにすることができた。上記の研究成果の一部を「訪問看護倫理における技術革新とケア」というタイトルで、ICU哲学研究会において、口頭発表し、技術とケアの媒介者としての訪問看護師像を伝えることができた。上記の研究成果の一部を、単著『生命倫理のレッスン』のなかに盛り込み、10代、20代の若い世代を中心とした一般の人びとに、現代医療の技術革新と人間性・ケアとの対立、および両者を媒介しコーディネイトすることが生命倫理の役割であることを伝えることができた。
