四国4県（徳島、香川、愛媛、高知）の周産期医療提供体制の把握を目的として分娩施設の現状を調査し、集約化の可能性を検討した。おもな結果は以下のとおりである：第1に、周産期医療提供体制について、総合周産期母子医療センターと地域周産期母子医療センター（以下、センター）は徳島県4施設、香川県3施設、愛媛県6施設、高知県2施設である。第2に、（1）2次医療圏での移動距離の平均および1出生当たり移動の機会距離は、分娩施設が高度化するにつれ長くなる。（2）対象とする93市町村について、最寄りの総合センターは5施設である。第3に、分娩施設の集約化の可能性を検討すると、四国4県のセンター15施設のうち12施設は医師数についての「ある望ましいレベル」に達しなかった。この場合、同じ2次医療圏内に複数以上のセンターや公的・大学病院があると、移動距離の延長を伴わない集約化が可能である。また、人口が少ない都道府県では、分娩を実施する病院のほとんどが公的・大学病院であるという傾向が見られる。それは、産科を提供する主体のうち、個人または法人立の医療機関がその2次医療圏や都道府県（3次医療圏）の産科医療提供市場から退出し、公的な供給主体だけが残っている状態とみることが可能である。2021年度より開始した、産後1年未満者を対象とするアンケート調査は、Covid-19流行下における褥婦の産後うつと産後ケア事業利用との関連の比較検討や、Covid-19が妊産褥婦に与えた影響についての実態調査などを目的とする。今後、結果の分析を進める予定である。
