项目摘要

2022年度は、在宅生活を送っている要介護高齢者のADL改善・重度化防止の観点から、通所介護事業所を拠点にする方策について、引き続き検討した。2021年度に実施した全国調査により、直近6ヶ月間で利用者のADL低下を認識している事業所が79.5%、ADL低下防止の目的で利用日を増加させた経験がある事業所は75.8%、そのうち85.2%の事業所が、利用日増加によってADL低下を防止できたことが明らかになった。一方で、多くの通所介護サービス利用者は、週に1回か2回の利用回数であり、利用日以外の情報が、ADL改善や重度化予防に重要であることも明らかになってきた。本年度は、COVID-19の影響で、実践現場を訪問しての調査はできなかったが、リモート会議方式で、ケアマネジャーや通所介護事業所の職員から意見を聞き、通所介護事業所を拠点にした、要介護高齢者の在宅生活継続支援について、引き続き検討した。本年度実施したヒアリングの結果、在宅生活を送っている要介護高齢者の多くが、複数の介護保険サービスを利用していることが分かった。当初は、在宅生活を送っている要介護高齢者のADL低下を予見した場合、スケジュールを変更して、通所介護サービスを前倒し利用することを検討していたが、要介護高齢者の状態や、居住地域における居宅介護サービスの状況によっては、通所介護サービスに限定せずに、他の居宅介護サービスの利用で対応した方が、より効果が期待できると推測された。そこで、2022年度から新たに始めた研究では、通所介護事業所のみならず、複数の居宅介護サービス事業者の情報を一元管理するシステムの開発を目指している。ただし、情報が増えることにより、効率的な分析が難しくなるという問題もあることから、通所介護事業所を拠点にする方法についても、継続して、その実現可能性を探索している。
