少数個転移に留まる肺癌（オリゴメタスターシス肺癌）は、遠隔転移を伴っているものの、外科切除によって完治する可能性があることが近年明らかになってきた。一方、当初、少数個転移であったが、経過中に複数臓器へ多数個転移を示す症例もあり、治療開始時点でこれらを峻別可能とする指標の確立は、Ⅳ期肺癌の適切な治療選択・予後向上のための、新たな突破口となる可能性がある。本研究の目的は、申請者がこれまで多施設共同研究で集積した少数個転移・多数個転移肺癌の手術材料を用いて、少数個転移肺癌に特有のmicroRNA発現特性を明らかにする事である。令和元年度は、肺癌手術材料から①少数個転移肺癌群、②多数個転移肺癌群を、転移臓器数、転移個数、臨床経過などを考慮し、解析対象として適したものをそれぞれ選抜した。組織型に関しては、当初非小細胞肺癌としての解析を予定していたため、腺癌、扁平上皮癌、腺扁平上皮癌、多形癌など多様な組織型が解析対象に含まれていたが、研究対象の背景因子を均一化するため、腺癌症例のみを解析対象とする方針とした。各群の解析対象症例を選抜した後、各症例の組織亜型、組織学的悪性度、病理病期、脈管浸潤等の病理組織形態学的な解析を行った。また、各症例における予後の追跡調査も併せて行い、各群毎の予後解析も行った。また、選抜した肺癌手術材料からTotal RNAを抽出し、バイオアナライザーを用いた品質評価を行った。令和2年度は、これまでに肺癌手術材料から抽出したTotal RNAを用いて、microRNAの測定作業を行った。microRNAの測定は3D-geneを用いて測定を行い、全症例の測定が終了した。令和３・４年度は、microRNAの測定結果の検証、および統計解析作業を行った。

The purpose of this study is to investigate the use of surgical materials for a small number of metastatic lung cancers and to identify microRNA expression characteristics unique to a small number of metastatic lung cancers. In this paper, the author considers the following factors: ① a few metastatic lung cancer groups, ② a large number of metastatic lung cancer groups, the number of metastatic tumors, the number of metastatic tumors, the number of clinical trials, the analysis of the target, and the selection of appropriate surgical materials. Histological analysis of primary non-small cell lung cancer (NSCLC) includes homogenization of background factors for histologic analysis of adenocarcinoma, squamous cell carcinoma, adenosquamous carcinoma, and pleomorphic carcinoma. After selecting each group of cases, the histological type, histological degree, pathological stage and vascular infiltration of each case were analyzed. For each case, the follow-up investigation shall be carried out in combination with the analysis of each group. Select lung cancer surgical materials from Total RNA extraction to quality evaluation. In 2002, the Total RNA and microRNA assays for lung cancer surgical materials were carried out. The microRNA assay was performed in the middle of the 3D-gene assay, and the assay was completed in all cases. The results of microRNA assay were verified and analyzed statistically in March and April.
