アルコール依存症患者は免疫不全状態であるため、致命的な感染症を起こす事が多い。断酒は根本的な治療であるが、精神的苦痛と肉体的困難を伴うため、その完結が非常に難しい。その為、断酒と並行して患者の感染抵抗性を回復させる別の治療法が必要である。アルコール依存症患者における免疫不全の原因の一つとしてアルコール依存症患者の末梢血中に認められるM2b 単球の存在が挙げられる。このM2b単球は、自らは抗菌活性を持たないだけではなく、抗菌活性を持つM1単球の発生を抑制することにより、宿主の感染免疫を抑制する事が判明している。しかし、現在までなぜこのM2b単球が出現するのかに関しては不明であった。最近、我々はこのM2b単球を誘導する新たな細胞（CD19+CD10-細胞）を発見した。この細胞の培養上清にて静止期の単球を刺激したところ、容易にM2b単球が誘導されてしまう。その培養上清に含まれるM2b単球の誘導因子がないかについて検討したところ、その上清にはHMGB1が含まれている事が判明した。このHMGB1はM2b単球の誘導因子ではないかと想定しており、今後その誘導機序の詳細を明らかにする予定である。また、その一方アルコール依存症に対しては断酒が基本的な治療にあるが、この断酒により、感染抵抗性が改善する事、またそれはM2b単球が減ることによる事が予想されているが、それにこのHMGB1が関与しているのかについても今後検討するつもりである。
