睡眠時ブラキシズム（Sleep Bruxism: SB）は、歯の食いしばり等を伴うため、歯科治療を困難にする要因であり、睡眠の質の低下など国民の健康維持の妨げとなる。その為、SBの機序を解明することは重要である。また、我々はこれまでSB が胃食道逆流や食道内への酸刺激によって引き起こされるなどSB と胃酸逆流との関連性が明らかにしてきた。一方、Helicobacter pylori（H. pylori）の除菌治療を行うと胃酸分泌能が回復し、一部のヒトでは逆流性食道炎を生じることがある。また、主な胃常在細菌であるH. pylori 以外に、Streptococcus mitis（S. mitis）など耐酸性の細菌が存在することが示されH.pylori 以外にも胃常在細菌が存在する可能性が示唆されている。そこで、本研究ではH. pylori の除菌治療前後におけるブラキシズムの自覚症状の有無や胃酸分泌量、H.pylori を含む胃常在細菌叢および口腔細菌叢の変化を解析し、各細菌叢とSB との関連性を検証する。本研究は胃・口腔常細菌叢とSB との関連性を解明し、SB の制御や胃癌などの疾患に対するオーダーメイド治療の基盤を目的とする。次の５項目を予定する。①：上部消化管内視鏡検査が必要な被験者から、胃粘膜と胃液および唾液を採取する。また、SB の有無を確認する。②：採取した試料のメタゲノム解析を行う。③：H. pylori の有無とSB の有無で口腔と胃内常在細菌叢を比較する。④：H. pylori 陽性の被験者の除菌療法前後の細菌叢を比較し、同時にSB の有無でも比較・検証する。⑤：①～④の結果から、口腔と胃内常在細菌叢をどのように維持・構築すると、SB や胃癌等の発症を抑制できるかを検討する。現在、被験者の選定中である。
