肝疾患の治療目標は、進行期肝疾患である肝硬変、肝がんへの病態の進展を抑制することである。近年、B型慢性肝炎、C型慢性肝炎ともに、抗ウイルス療法の治療効果は向上し、さらに、肝炎対策の推進、および生活習慣病の増加等のため、肝硬変・肝がんの成因となる背景肝疾患の構成は大きく変化してきている。肝疾患の実態と成因の推移、肝がんを合併する背景肝疾患の成因の推移等の記述疫学のほか、処方等の診療行為、肝がんに関連する肝疾患以外のその他の併存症、また、特定健診等情報データベースにより、肝機能障害、生活習慣質問票とその後の肝がん発生との関連を検討することにより、肝がんの新たなリスク因子を解明するそのため、本研究では、悉皆性の高い医療保険データベースを用いて、リアルタイムに肝疾患の実態を把握し、肝がんの新たなリスク因子を解明する。初年度は、市販医療保険データベースにおいて、今後の生活習慣を基礎とする肝がんの成因増加への対策に資するよう、肝疾患、肝病態、診療行為、健診項等について、変数の定義を行った。2年度目以降市販医療保険データベースの拡充に伴い、データの追加のうえ、再度変数定義の検討、対象の抽出を行い、解析用のデータセットを作成した。解析環境を整備し、肝疾患の実態と成因の推移、 肝がんを合併する背景肝疾患の成因の推移、薬剤等の診療行為・肝がんに関連する肝疾患以外のその他の併存症等と肝疾患および肝がんとの関連を検討している。また、本課題の方針決定のため実施した抗ウイルス療法と肝癌リスクに関するメタアナリシスについて論文受理された。
