项目摘要

2022年度は、2020年度および2021年度に引き続き、研究対象としていた外洋小離島への訪問が新型コロナウィルス感染症の影響で制限されたことから、計画していた離島住民を対象とした「人生会議」の実施と、それに伴う調査を行うことができなかった。しかし、へき地診療所で働く看護職8名を対象に、無医島に居住する慢性疾患患者に対する看護師の支援と葛藤について、オンラインにて聞き取り調査を行った。2019年度に構築されたへき地診療所看護師の継続教育支援ネットワークの会議は、各島をWebでつなぎ、鹿児島市内で2023年3月22日に開催された。2019年度からへき地診療所の看護師を2名体制としたことで、師長会が設立されるなど、各島が連携したヘルスケアの組織化が進んでいる。各へき地診療所で作成されてきた業務マニュアルは、「看護基準マニュアル」として標準化することになった。また、師長会設立に関する課題、診療所目標の設定、住民支援と人材派遣の取組について報告され、検討を行った。離島において喫緊の課題となった新型コロナウィルス感染症対策については、2020年度から調査を開始し、2021年度は、全島が新型コロナウィルス感染症のクラスターと認定された離島の保健師を対象とした調査をまとめ学会誌に投稿し、掲載された。また、2021年度に当該離島において、新型コロナウィルス感染症に罹患した経験を持つ住民7名を対象に行った9項目からなるインタビュー調査については、海外誌への投稿に向けて、現在執筆中である。
