项目摘要

1.研究目的:病院における診療記録開示の状況と看護記録の実態を調査・分析し、チーム医療の促進・患者等への情報開示、インフォームドコンセント等の視点からその間題点を抽出して、診療情報の開示に向けて看護記録はどうあるべきかの提案をする。2.研究方法診療記録開示に積極的に取り組んでいると言われているK病院、S病院、C病院を訪問し、診療記録開示の実際を看護部長へのインタビューと病院見学を通しての調査を行い、その結果と、東京都立病院、国立病院、国立大学附属病院、日本医師会のガイドラインを基にして比較検討を行った。さらに、アメリカにおける診療記録開示の現状を把握するため、マサチューセッツ州のボストンに位置するハーバード大メディカルスクールの教育病院である5施設を訪問し、患者の権利および看護記録を含む診療記録についての調査を行い、日本とアメリカにおける診療記録開示の状況について比較を行った。3.結果:診療情報や診療記録は患者の個人情報であることから、それらを開示することは患者の診療情報の自己コントロール・アクセス権を認めるという点に最大の意義があり、そのためにも、アメリカのような患者の権利に対する法的な整備は必要不可欠であるといえる。看護記録が開示に耐えうるためには、客観的な記載がなされる必要があり、看護職者には、常日頃からそのような看護記録を書く訓練が必要である。専門用語等患者がわからない語彙等については、説明を加えながら開示をすることが望ましい。また、看護記録のあり方について看護職者間で共通の認識を持つために、施設毎で基準やガイドラインを作成したり、監査システムを確立させたりする必要がある。看護記録の様式は、重要な事項を落とすことなく簡潔に書けるものでなければならず、チーム医療を促進し、よりよいケアが提供されるためにも他職種と共有する形が望まれる。その場合、電子記録は適している方法と言える。看護職者が「自律した専門職」としてその専門性を発揮するためにも、看護記録の開示は有効であり、必要である。その意昧でも、看護記録の法的整備の早期実現が望まれる。
