令和4年度は、がん性創傷のにおい管理に関するアセスメントとケアの現状を明らかにすることを目的に、がん性創傷のケアの臨床経験が豊富な皮膚・排泄ケア認定看護師12名を対象としたフォーカスグループインタビューを実施した。全国的に新型コロナウイルス感染症が拡大した時期であったため、3グループに分け、90分／回、計3回のWEB会議システムを用いたインタビュー調査を行った。研究デザインは質的記述的研究とした。調査結果はすべて逐語録に起こし分析中だが、主に以下のことが明らかになった。におい管理は滲出液のコントロールと洗浄が重要であること、外用薬・消臭剤・吸収パッドなどを組み合わせて使用すること、空間消臭や廃棄物処理にも配慮すること、患者の生活環境や心理面を重視した全人的なアプローチが重要であることが示された。国内学会の交流集会でがん性創傷のにおいと滲出液の管理に関するテーマを企画し、本調査結果の一部を発表した。参加者とのディスカッションから、がん性創傷をもちながら在宅生活を送る人は多く、本人と家族および訪問看護師が難渋するケアのひとつであることが再認識できた。本研究のゴールは、局所管理にとどまらず、環境管理、心理的ケアをふまえたがん性創傷のにおい管理に関するマネジメントモデルを作成することである。今回の調査結果を論文として公表するとともに、がん性創傷のにおい管理に関する在宅版マネジメントモデルを構築し、容易に活用できる情報提供ツールを作成する予定である。
