项目摘要

2022年度は、5県に在住する小児1型糖尿病患者を対象に行った「成人型医療への移行の準備状態(レディネス)に関する無記名自記式質問紙調査」の分析を完了させた。12歳から24歳の小児期発症1型糖尿病患者の成人期医療への移行レディネスについて、内科に転科済みの患者の特徴とレディネスの関連要因を明らかにした。調査内容は、成人期医療への移行レディネス、糖尿病自己管理状況、病気の受け止め方、糖尿病自己効力感であり、合計69名の患者から回答が得られた。レディネスが形成されていたのは、内科に転科済みの患者は11名中4名（36.4％）、小児科通院中の患者では58名中14名（24.1％）であった。レディネスに関する『知識』と『行動』の総得点の中央値は、いずれも内科に転科済みの患者の方が高かったが、有意差は認められなかった。また、成人期医療への移行に関するレディネスの関連要因として、糖尿病キャンプの参加回数、年齢、罹病期間であることが新たに明らかになった。レディネスは、内科に転科する前に形成されるべきだが、形成できていない現状があることから、小児科での移行の準備を整えるための支援の充実が急務である。今後、具体的な支援として、小児科側がレディネスに関する知識を提供する準備を整え、多職種と協働し、継続的・段階的に知識の学びをサポートできる支援や、移行パンフレットの充実、そして、小児糖尿病キャンプへの参加促進等を行っていくことが必要である。なお、本研究結果を、日本糖尿病学会誌「糖尿病」に投稿し、2023年4月に原著論文として採択された。

In 2022, we completed the analysis of the anonymous questionnaire survey on the status of preparedness for the transition of adult medical care in children with type 1 diabetes in May. The characteristics of patients with type 1 diabetes in childhood and adulthood and the main causes of their development in internal medicine have been clarified. 69 patients in total answered the questions of the survey, including the transition of adult medical care, diabetes management status, treatment of diabetes, and diabetes self-efficacy. 4 out of 11 (36.4%) patients in internal medicine department and 14 out of 58 (24.1%) patients in general pediatrics department were diagnosed with acute respiratory syndrome. The central value of the "knowledge" and "action" related to the development of medicine is high, and the patient's intention is low. The main causes of diabetes mellitus are the number of years of participation, the length of illness, and the number of years of illness. The preparation for the migration of small subjects is urgent. In the future, specific support, small family related knowledge, preparation, multi-professional cooperation, high-level knowledge, support, migration, small family related knowledge, participation promotion, etc. The results of this study were submitted to the Journal of the Japanese Diabetes Association, Diabetes, April 2023.
