项目摘要

近年，超高齢化や医療の高度化に伴い，手術療法を受ける高齢者は増加し，術後せん妄（以下 POD）の発症が問題となっている。PODは，術後経過のみならず予後にも影響を及ぼす深刻な問題である。そのため，患者の安全を第一と考え，やむを得ない場合との判断で身体拘束が実施される場合が多いが，身体拘束は人権侵害を引き起こすリスクがある。我々は，これまでに若年健常者および高齢者，さらに高血圧治療中の高齢者や緩和ケアを受けている入院患者などにおいて，生理学・心理学的にハンドマッサージ法のリラクセーション効果や社会的相互作用を明らかにし，その安全性も確認できた。そこで今回は，ハンドマッサージ法をPOD発症の促進因子である不快症状や不眠，不安などの緩和を目的として，多職種連携チームによる多因子介入のひとつとなることを目指したいと考えている。2019～2021年度は，COVID-19の感染拡大により病院施設での研究活動が中断され，予定していた臨床での介入研究が実施できなくなった。そこで，高齢患者のPOD妄予防および症状緩和を目的とした多職種連携チームによる，より効果的な多因子介入について文献検討を実施し，介入の一つとしてハンドマッサージ法の在り方を検討してきた。そして，新たにせん妄ハイリスク高齢患者の入院生活を支援する多職種連携チームが活動している病院との関係を構築し，そのチームの活動に参加し，文献検討での考察をさらに充実させ，文献検討で明らかにされた効果をどのように臨床場面に適応し，課題をどのように解決していけるかを検討してきた。チーム医療において患者やその家族もチームの一員であり，患者・家族も実施される医療についての知識が求められている。そこで本年度は，中高年者を対象にPODの認知度を調査した。加えて，PODを疑似体験してもらい，取り得る態度と個人の特性との関連を検討している。
