共生型サービスは、介護保険および障害福祉サービスにおいて、高齢者、障害児者などの多様な利用者に対して、同一の事業所で一体的にサービスを提供する。特区や基準該当により同様のサービスは既に部分的には導入されていたが、2018年に介護保険制度および障害福祉サービスに新たなサービス類型として創設された。本研究は、地域に住む人々の生活構造を踏まえて、この共生型サービスがいかに普及していくのかの様相を追い、課題を探ることを目的とする。2022年度も引き続き、新型コロナウィルス感染症の影響による福祉施設への立ち入り制限や研究代表者の体調不良のため、予定していた共生型サービスに関する独自の質問紙調査を実施することができなかった。そのため、厚生労働省やその補助事業を通じた既存の調査結果について、関係者への聞き取りや文献調査を中心に進め、実現可能な研究の方向性について再検討をおこなった。2022年11月現在、共生型サービスの普及はかなり限定的であり、障害福祉サービス事業所で共生型介護保険サービスの指定を受ける事業所は全国で179ヶ所、介護保険サービス事業所で共生型障害福祉サービスの指定を受けるのは1056ヶ所に過ぎない。さらに、共生型サービスの指定事業所は必ずしも公表されていないことから、地理的な普及状況について把握することも困難であった。そのため、本研究では質問紙調査による実態把握は取り止め、残りの研究期間で参与観察やインタビューによる事例検討を積み重ねていくこととした。また、生活構造に関する理論的検討や既存の住民調査やサービス従事者調査の二次分析を進め、現代社会で求められる支援のあり方についての検討を進めた。その結果、要介護度が重いほど家族介護者の不安や負担が重い訳では必ずしもなく、置かれた状況に応じて異なるニーズにどこまでどのように応えていくのかが課題となることが示唆された。
