本研究の目的は、看多機における認知症高齢者の終末期ケアに対する職員の意識やその実態を明らかにすると共に、今後の認知症高齢者の看取りへの可能性について検討する。研究対象と方法は、本州45都道府県(日本の人口約86％占める)の看多機をWAM-NETで検索し、785事業所概要(中部圏127事業所、中部圏以外598事業所)から基本情報を得た後、看護師の代表者にアンケートを発送し、郵送による自記式質問紙調査行った調査内容は、認知症日常生活自立度Ⅱ以上の人の受け入れ状況や重度化する利用者への対応や看取りの実績、認知症終末期ケアに対する取り組みやケアの実地状況(実態)等の記述統計を行ったのち、認知症高齢者看取り件数を目的変数（ｙ）として、説明変数（ｘｉ）を①施設種別、②併設ステーションの有無、③看護職数（登録者数に対する看護師比率）、④登録者数における介護職数（介護職比率）、⑤ターミナル加算、⑥認知症看取りへの意見、⑦生前意思表明書の導入の有無による関係について平均値における有意差検定を行った。結果は、有効回答数は165人(有効回答率21％)、地域別では、中部圏は31人（24.4％）、全国134（22.4％）であった。利用者の6―7割が認知症を有している人で占められていた。事業所概要では①施設種別、②併設ステーションの有無、④登録者数における介護職比率、⑦生前意思表明書の導入の有無に関しては、認知症看取り件数に有意差が見られなかった。認知症看取り件数と有意差があった項目は、③看護職数、⑤ターミナル加算、⑥認知症看取りの意見への関係が明らかになった。結論として、看多機の認知症高齢者の終末期ケアに関しては、管理者の積極関な姿勢が看護師の雇用も多くなり、また、ターミナル加算においても積極的な取り組みを行うことが可能になっており、看取りの可能性を拡大させていることが明らかになった。
