项目摘要

若年健常者における鼻腔・咽頭の黄色ブドウ球菌保菌実態を明らかにすることを目的に、臨床検査学または口腔保健学を専攻する3・4年生を対象に、鼻腔と咽頭の保菌調査ならびに口腔保健行動を主とする質問紙調査を実施した。また、保菌者1名につき各部位から1株分離した菌株について、黄色ブドウ球菌の付着因子ファイブロネクチン結合タンパク(fibronectin-binding protein, FnBP)遺伝子の塩基配列の多型に基づくFnBP sequence typing (FnST)を、本研究室で開発した21タイプに型別可能なmultiplex-PCR法にて実施した。FnSTはゲノム系統を反映したMLSTに対応することがわかっている。保菌調査では313名中、鼻腔保菌の116名(37%)に比べ咽頭保菌178名(57%)で有意に保菌率が高かった(OR:2.24, 95%CI:1.62-3.09)。鼻腔と咽頭両方に保菌する81名中、31名(38%)が異なる型の菌株を保有していた。鼻腔と咽頭由来株のFnST型分布には偏りが見られず、保菌にかかる菌側の因子は両部位で差がないことが推測された。質問紙調査で咽頭保菌のリスク低減要因として有意差が得られた項目は、口腔保健学専攻(OR:0.30, 95%CI:0.19-0.49）、デンタルフロスの使用(OR:0.38, 95%CI:0.23-0.63)、歯磨き1日3回以上(OR:0.50, 95%CI:0.31- 0.79)であった。一方、鼻腔保菌は口腔保健行動により影響されず、リスク上昇要因はアトピー性皮膚炎(OR:2.07, 95%CI:1.06-4.04)と食物アレルギー(OR:2.83, 95%CI:1.31-6.12)の既往であった。これらの結果から、黄色ブドウ球菌の鼻腔と咽頭の保菌は互いに影響は受けるものの独立した現象で、保菌のしやすさの要因は鼻腔と咽頭で異なるが宿主側にあること、咽頭保菌は口腔保健行動による影響を受けやすいことが示唆された。
