项目摘要

在宅療養中の高齢者が充実したエンドオブライフを送るためには、日常の療養生活を支えている訪問看護師がアドバンス・ケア・プランニングを行うことが有用と考える。訪問看護師がアドバンス・ケア・プランニングを推進するための介入プログラムを開発し、有効性・普及可能性について検討する。令和4年度は、前年度より開始した教育プログラムの1つであるACPセミナー受講者を対象に、フォローアップ面談を実施した。参加希望者は6名で、セミナーの評価に関する質問のほか、実際のACP場面における不明な点や困りごとについてアドバイスを行った。セミナー受講後のACP実践状況では、参加者全員が、継続的な対話を意識しACPを行っていた。コミュニケーションの基本を心がけ、その都度声がけを行い継続的な対話に結びつけたことが語られた。また、こちらが聞きたいことを聞くのではなく、利用者が話したいことを聞くことを心がけ、回を重ねて対話する時間が大切であると感じたことが語られた。意識的に対話することから継続的な対話の必要性を感じており、特定の疾患に限らず、すべての疾患においてACPが必要となるため継続的な対話のトレーニングを行いたいなどの意見が寄せられた。
