项目摘要

誤嚥性肺炎の予防の重要な要素の一つとして随意的な咳嗽力 (cough peak flow: CPF)がある。CPFの低下は誤嚥性肺炎の危険因子の一つである。咳嗽には深吸気における吸気筋の筋力と胸腔内圧の上昇ならびに爆発的な呼気に呼気筋の筋力が必要と考えられている。CPFは随意的な咳嗽のflowであるため、通常の咳嗽と同様に呼吸筋筋力が影響すると考えられている。近年、四肢の筋力同様に高齢者における呼吸筋筋力にもサルコペニアが起こると報告されている。呼吸筋筋力の低下に伴いCPFが低下することが予想されるものの、地域在住高齢者におけるCPFの報告は非常に少なく、CPFに関する観察研究を散見する程度である。介入研究はほとんどなく、CPFを向上させるトレーニング方法は確立されていない。そのため本研究はCPFに影響を与える要因を明らかにすること、さらにCPFを向上させるトレーニング方法を確立することを目的としている。本研究では、2018年度に地域在住高齢者のCPFに影響を与える要因に関する研究を実施し、CPFには先行研究で報告されていた呼吸筋筋力だけでなく、腹筋筋力も影響を与えることを明らかにした。しかしながら本研究はスパイロメトリー、呼吸筋筋力評価、CPFの評価といった飛沫感染リスクが高い評価を行うものであり、また対象も重症化リスクの高い地域在住高齢者としているため、2019年度以降研究の実施を見合わせており、流行2022年度もこれまで同様に新型コロナウィルス感染症のために研究をストップさせており、本研究は感染状況を踏まえ実施再開のタイミングをうかがうこととなった。
