本研究の目的は、災害時の精神科入院患者に対する精神症状のトリアージ（医療の優先順位をつける選別方法）の現状と、今後、災害時に看護師が精神科入院患者に対する精神症状のトリアージ（以下、精神症状トリアージ）として優先すべきと考える項目について、精神科病院の看護管理者への調査から明らかにする。阪神淡路大震災以降に大災害があった地域を中心に日本精神科病院協会に登録されている精神科病院262施設の看護管理職者を対象に、自由記述式の質問紙調査（郵送法）を行った。調査期間は2023年2月～3月。調査内容は、設置主体、病床数、看護職員数、被災経験の有無、被災の内容、現在取り組んでいる精神症状のトリアージ対応（マニュアル等）の現状、今後、精神症状トリアージとして優先すべきと考える項目とその理由について調査した。結果として、回収率は20.2％（53施設）で有効回答率は100％であった。被災経験がある施設は27施設（50.9％）で主に地震災害（21施設）が多かった。被災経験の有無に問わず、現在災害時に精神症状のトリアージ対応を実施している施設はなかった。精神症状トリアージとして優先すべきと考える上位項目は、被災経験ありの施設で自傷他害（16件）、興奮（15件）、暴力（12件）、無断離院（11件）、幻覚妄想（9件）、被災経験なしの施設では自傷他害（15件）、興奮（14件）、暴力（13件）、幻覚妄想（11件）無断離院（9件）であった。それぞれの項目の理由として、自傷他害は【被災による精神症状悪化の影響】【被災による興奮状態の影響】、興奮は【被災により精神症状が不安定になる影響】、暴力は【被災による焦燥感・興奮の影響】、無断離院は【精神疾患による危機判断不足】【精神疾患による避難の非協力的態度】【入院形態に合わせた患者状況把握の必要性】、幻覚妄想は【被災による症状悪化・暴力の防止】であった。
