甲状腺乳頭癌中リスク症例では、甲状腺全摘術あるいは葉切除術のいずれも許容されるがその適応を決定する指標は存在しない。そのため現状としては、血清サイログロブリンによる術後の再発モニタリングを容易にするために、甲状腺全摘術が行われることが多い。しかし、甲状腺葉切除術と比べ甲状腺全摘術では患者の術後のQOLは著しく低下する。その状況を打開するためより有益なリスク分類を確立するとともに、甲状腺葉切除術後でも有用な腫瘍マーカーを開発することが求められる。本研究は、metabolic tumor volumeに基づく新たなリスク分類を構築し、liquid biopsyで検出する循環血液中のBRAFV600E変異を腫瘍マーカーとして確立することを目的としている。本年度は前年度と同様、研究対象となる患者の手術検体ならびに血液検体の収集を行い、また同症例の術前PET-CT画像データを収集・保管し、画像解析を行ってmetabolic tumor volumeを算出した。

Thyroid papilla cancer is a disease that can be determined by total thyroidectomy. The status quo of thyroid cancer is very common in patients with thyroid cancer. The patient's QOL after thyroidectomy is lower than that after total thyroidectomy. The condition of the thyroid gland is open. The classification of the thyroid gland is open. The purpose of this study is to establish a new classification of metabolic-tumor volume and to detect the presence of BRAFV600 in circulating blood. This year, compared with the previous year, the collection of surgical specimens and blood specimens of patients with the same disease was performed, and the collection, storage and analysis of preoperative PET-CT images were performed.
