大規模診療情報データベースが整備され、リアルワールドデータを用いた医学研究への応用が期待されている。この研究活用にあたっては、データベースに記録された疾患名や治療内容の情報が正確に記載されていることが肝要である。しかし、情報の妥当性を検証した研究（バリデーション研究）は少ない。特に悪性腫瘍は、レセプト等により実際の患者の状態が十分に記述できるかが懸念される疾患領域である。本研究は、独立行政法人国立病院機構に所属する多施設において診療録調査を実施し、その結果を基準とすることで、データベースに記録された悪性腫瘍の診断と治療内容の情報の妥当性を検証するものである。国立病院機構は全国の各施設からDPCデータ、レセプトデータおよび電子カルテデータを収集している。本研究は肺癌、大腸癌、胃癌、肝癌、悪性リンパ腫のいずれかで入院し治療を受けた患者を対象とし、病変部位、進行度、術式、化学療法、全身状態などを調査する。調査に先立ち、本邦で実施されているバリデーション研究の現況を把握するために文献レビューを実施し報告した。2022年までに発表された36編のバリデーション研究が特定され、近年は増加傾向ではあるものの小規模な研究が多いという結果であった。多施設・大規模なバリデーション研究の重要性が改めて明らかとなった。2022年度まで、新型コロナウイルス感染症による協力施設の負担等を考慮してカルテ調査を見送ったが、既に複数の医療機関から調査への承諾を得ている。2023年度はさらに協力施設を募集した上で調査を実施する予定である。
