项目摘要

本研究の目的は、転移・再発乳がん患者のケアに従事する外来看護師が認識している経済毒性に関する看護の役割と看護支援の現状を明らかにすることであり、現状の看護実践を踏まえて、経済毒性をマネジメントするための看護支援ガイドを考案することを目標としている。2022年度は実態調査に向けて研究計画を立案し、倫理審査委員会の承認を得て調査を開始した。医療機関7施設から協力が得られ、がん看護に従事する看護師約500名を対象とした無記名自記式質問紙調査を実施している。調査項目は、経済毒性に関する役割認識、実践状況、実践に対する阻害要因等とし、文献検討をもとに精選した。看護師は患者にとって身近な支援者であり、患者の生活をよく知る強みを生かして経済毒性に気付き、必要なリソースにつなぐ役割が期待されていると考えられる。そのため、前述した調査項目は、相談窓口としての情報発信やアセスメント、多職種連携に関する内容を中心に構成した。本調査により、看護師自身が認識している役割の内容や現在提供されている看護実践を明確にするとともに、役割認識と実践の頻度に乖離がある項目および看護実践を妨げる要因を同定する。現状の実践を基盤として、役割認識の変容や看護実践の強化が必要な内容を明確にしていく予定である。あわせて、がん種やセッティング(外来/病棟)による特徴を見出し、転移・再発乳がん患者のケアに活用できる看護支援ガイドの作成に活用する。
