本研究は、基底細胞癌の各病型について、特に斑状強皮症型の臨床的、病理組織学的な表現型形成に関与する遺伝子および遺伝子群の同定、さらにその背景に存在する遺伝子変異プロファイル検証のための基礎的データを確立し、基底細胞癌の各病型の病態や発症機序の相違を明らかにすることを目的である。これまでの予備解析に使用した斑状強皮症型と結節型基底細胞癌の症例に加え、更に症例数を増やし、現時点で合計20検体の症例の収集が終了した。それらのトランスクリプトームシーケンスを行い、解析を進めている。これまでの予備解析で有意に遺伝子発現変動が確認されたコラーゲンおよびMMP遺伝子ファミリーに加え、いくつかの特徴的な遺伝子ファミリーの発現変動が確認された。これらの遺伝子は、これまでに腫瘍の悪性度と関連が示唆されていることから、斑状強皮症型基底細胞癌の高病原性と関連があることが推測される。これらの遺伝子のタンパクレベルの発現変動を確認するため、当該遺伝子ファミリーのいくつかの遺伝子に着目し、斑状強皮症型基底細胞癌の臨床組織検体を使い免疫染色をすすめている。いくつかの遺伝子については免疫染色でタンパクの発現変動が確認された。これらの異なる基底細胞癌の病型の群間に置いて、特徴的な遺伝子発現変動が見られる現象は、基底細胞癌の遺伝子変異パターンの差異に起因すると考えられる。この差異を明らかにするために、腫瘍のゲノム配列をエクソーム解析により明らかにする。まず、各病型合わせて15症例の基底細胞癌症例において、３９の遺伝子に着目し、変異を同定するためのシーケンを終了している。今後、2群間で特徴的な遺伝子変異パータンが存在するか検証し、各病型の表現型との関連を検討する。

The purpose of this study is to establish the basis for the identification and identification of genetic variants and genetic subgroups related to the development of clinical and histopathological phenotypes of basal cell carcinoma, especially spotted scleroderma, and to clarify the correlation between the pathological mechanisms of basal cell carcinoma and its various disease types. The number of cases of nodular basal cell carcinoma (BCC) and nodular scleroderma (SCC) has increased, and the total number of cases of BCC at present is 20. All kinds of products are sold in China. This analysis confirms the occurrence of intentional gene changes and MMP gene changes. The high pathogenicity of scleroderma basal-cell carcinoma is presumed to be related to the high pathogenicity of scleroderma. Immunostaining was performed on clinical tissue samples of scleroderma type basal cell carcinoma when the expression of the gene was detected. In the case of immunostaining, it is confirmed that there is a change in the expression of the protein. This phenomenon is related to the differences between the types and characteristics of basal cell carcinoma and the causes of these differences. The difference between the two groups is clear. The results showed that there were 15 cases of basal cell carcinoma, 39 cases of basal cell carcinoma, and 15 cases of basal cell carcinoma. In the future, there will be differences in the characteristics of the two groups, and the relationship between the phenotypes will be discussed.
