项目摘要

ANCA関連血管炎は全身の諸臓器を障害する難治性疾患で、本邦では高齢者を中心に近年増加傾向にあり、その臨床像は多様性に富む。COVID-19パンデミック下で不確実性が増している医療現場において、個々の患者や地域の特性に応じて診療を最適化するためのリアルワールドエビデンスが求められている。本研究では新たにANCA関連血管炎の疾患レジストリを構築し、病態解明や、患者特性に応じた個別化医療、パンデミック下における最適な治療戦略の提唱につながる知見を発信していくことを目的とする。本年度は、国内24の医療機関で2017年～2020年に診断した680症例についての診療情報を収集し、データベースの構築を行った。現在、構築されたデータベースを用いて分析と検証を開始している。一例として、ANCA関連血管炎の初期の死因として重要な感染症の低減につながる新たな知見として、治療後に生じる低IgG血症の意義について明らかにした。また、ANCA関連血管炎の臓器病変の一つである肥厚性硬膜炎について、合併割合や臨床像を明らかにし、論文化を行った。引き続き様々な臨床疑問についての分析を行っている。今後、2021年以降の症例についてもデータ収集を行っていくことで、パンデミック前後の比較も含む、多彩な臨床疑問を解決可能なリアルワールドデータベースが創出され、以てANCA関連血管炎の患者の生活の質の向上に資することを目指せると考えている。

ANCA-associated vasculitis is a refractory disease that affects all organs of the body, and there is an increasing trend in the number of patients in the country in recent years. COVID-19 infection is not accurate, the medical field is not accurate, the patient's geographical characteristics are not accurate, the diagnosis and treatment are optimized, and the patient's diagnosis and treatment are optimized. This study aims to provide information on the development of new ANCA-associated vasculitis disorders, the identification of pathologies, the characteristics of patients, the development of individualized care, and the development of optimal treatment strategies. This year, 24 medical institutions in the country have been diagnosed with 680 cases from 2017 to 2020. Now, the construction of the project is in progress. A case of ANCA-associated vasculitis and its early cause of death are important for the reduction of infectious diseases. The clinical manifestations of hypertrophic dural inflammation and ANCA-associated vasculitis are discussed in detail. The analysis of clinical questions is carried out in the middle of the study. In the future, in 2021, the number of cases will be reduced, and the comparison between before and after the treatment will be carried out. The clinical questions will be solved, and the quality of life of patients with ANCA-related vasculitis will be improved.
