研究を行うため、砕石位での手術を施行する症例を対象とした前向き観察研究を立案し、当施設の倫理委員会へ申請致しました(研究番号20-226：砕石手術における至適術中体位の検討)。当施設において砕石位を体位として手術を行う症例に対し、手術の際に体圧測定器を用いることで術中の荷重部位にかかる圧負荷を視覚的に、リアルタイムに評価することが可能となります。本研究では上記測定器を用いて術中の体位によりかかる圧負荷の経時的変化を測定しました。「かかる圧の経時的変化」の例として腹腔鏡下での直腸癌手術を挙げた場合、手術の際には砕石位の体位に加え右下頭低位にする必要があります。当科では先行研究として砕石位での腹腔鏡下手術の術中に左下腿にかかる外圧に関する解析を行いました。その結果、砕石位での術中に左下腿にかかる外圧は右下頭低位にすることで上昇し、水平位にすることで低下することを証明致しました。その結果に加え、砕石位での手術を行った症例の一部に仙骨部など左下腿以外の部位にも体位の影響と考えられる痛み、発赤などの症状を認めていたことが判明しました。そのため、今回の研究では測定対象を頭頚部、体感、四肢などに広げ、各荷重部位にかかる圧負荷の測定としました。同じ術中の体位でも左下腿以外も水平位と右下頭低位の状況での加わる圧負荷の違い、右下頭低位から水平位に戻すことで生じる圧負荷のに変化(特に除圧の方向に傾くか)、などを測定しました。倫理指針に遵守し、インフォームドコンセントを行い、同意を頂いた方の手術症例を集積しました。1年間研究を行い60症例の集積を終えました。有害事象も認めず、安全性も問題なく遂行致しました。現在得られた測定結果の解析を行っております。
