本研究では身体的、社会的脆弱性を有することの多い高齢消化器がん患者に対する手術治療の適応判断の一助とすべく、当施設の対象の臨床情報についてコホート分析を行いその結果を踏まえて一定期間の前向き観察研究を実行することにより、高齢消化器がん患者の術後遠隔期の自立性喪失のリスク因子を探索することを目的としている。2022年度は研究の第一段階として後向きのデータ収集、解析を企図した。当施設の過去の診療録を網羅的に検索し、当施設消化器外科にて2010年1月から2021年12月の期間に消化器領域の腫瘍性病変に対して根治手術を企図した3634症例を抽出した。これらを術式別に分類し、そのうち高侵襲手術が多い肝胆膵外科領域の手術症例553例について術式別に手術成績（術後合併症、術後在院日数等）に関する因子についての情報を収集した。65歳未満の若年者群と65-74歳の前期高齢者群、75歳以上の後期高齢者群の間において因子の比較解析を行った結果、Clavien-Dindo分類 Ⅱ以上の術後合併症に関して年齢区分と合併症発生率の間に明らかな統計学的な関連は認めなかったものの、年齢区分が高くなるに従い在院日数が長期化する傾向を示した。これは高齢者特有の家庭における住環境やバックアップ体制の脆弱性に起因する退院調整期間の長期化をみている可能性も想定されるが、今年度はそれらの社会的因子は解析にまで至らず次年度以降の検討予定としている。その他、2022年度内では限定的な術式かつ短期的な情報のみの解析を行うに留まっており、当初の研究目的達成のためには今後長期成績も併せて検討してゆく予定である。

This study was conducted to assess the physical and social vulnerability of the patient and to assess the clinical information available to the patient and to evaluate the results of the study. Objective: To explore the factors of loss of independence in patients with high digestive tract in the long postoperative period. In 2022, the first stage of the research was to collect and analyze the data. When the past records were collected, 3634 cases of digestive tract diseases were identified during January 2010 to December 2021. 553 surgical cases in the field of hepatobiliary surgery were classified according to the types of surgery, and the information related to the types of surgery (postoperative complications, postoperative hospital days, etc.) was collected. The results of comparative analysis of the factors between the 65-year-old group and the 65-74-year-old group and the 75-year-old group showed that the incidence of postoperative complications was related to Clavien-Dindo classification II and above. The statistical correlation between the incidence of complications and the age group showed that the number of days in hospital tended to increase over time. The analysis of factors affecting the long-term likelihood of retirement adjustment period due to the vulnerability of the family environment and the system unique to the elderly will be conducted in the next year. In 2022, we will focus on short-term information analysis, long-term results, and future research objectives.
