项目摘要

本研究の目的は、音楽家における演奏に関連する筋骨格系の障害(Playing-Related Musculoskeletal Disorders : PRMD)に関する疫学的調査を行い、国内の音楽家の上肢機能障害の実態を明らかにすることである。具体的には、手外科医師による上肢機能の検診を行い、フォーカルジストニアの早期発見(特に病院で診断されていないTubiana’s Scale 3-4)を目指している。ポータブルエコーを併用することで占拠性病変や構造異常、炎症所見の有無を確認、診断精度を向上させる工夫を行なっている。令和４年度は、検診に使用するエコー（Canon 超音波診断装置Viamo SV7）を購入した。また検診に用いる問診票および所見記録用紙について、研究カンファレンスにてディスカッションを行って完成させた。さらに、東京音楽大学と千葉大学間において研究連携の協定を締結した。これに基づき、まず探索的に2022年7月に東京音楽大学にてパフォーミングアーツ医学に関する講義を行なった際に、アンケート用紙による調査と身体診察を実施した。アンケートでは、性別・年齢・身長・体重・利き手・職業・音楽歴（専攻楽器・経験年数）、演奏頻度（時間/日・日/週、既往歴、PRMDの自覚の有無と症状のある部位、通院歴、痛みがある場合VAS(Visual Analogue Scale)等について調査した。合わせて国際的な上肢機能評価スコアである The Quick DASH（Disabilities of the Arm, Shoulder, and Hand）も記入していただいた。また、2023年2月に浜松国際ピアノアカデミーにおいて同様の講義を行い、身体診察を行った。なお当院観察研究倫理委員会に本研究に関する倫理審査を申請し、実施について承認を得た。取り組みについては学会等で発信している。

The purpose of this study is to investigate the epidemic of Playing-Related Musculoskeletal Disorders (PRMD) in Chinese musicians and to clarify the status of upper limb dysfunction in Chinese musicians. Specific findings include the diagnosis of upper limb function by hand surgeons, early detection of disability (especially hospital diagnosis), and Tubiana's Scale 3-4. It takes time to confirm the presence or absence of structural abnormalities and inflammatory findings, and to improve the accuracy of diagnosis. In 2004, the company purchased the Canon ultrasonic diagnostic device Viamo SV7. The examination was completed on the paper used for the examination and the paper used for the record of the findings. The agreement between Tokyo Music University and Chiba University was concluded.
