项目摘要

サイトメガロウイルス(CMV)前眼部感染症は、ぶどう膜炎や角膜内皮炎を来し、不可逆的な角膜浮腫や続発性緑内障による視神経萎縮により視機能喪失に至りうる疾患である。日和見感染症として免疫抑制状態の患者に発症するCMV網膜炎と異なり、健常者に発症することが特徴であり、その病態には不明な点が多い。これまでの国内外の報告や当院での経験で、CMV前眼部感染症は中高年の男性に好発するとされており、男性の割合は女性と比べて2-4.1倍多い。このことから、我々はCMV前眼部感染症の病態には性ホルモンなどに由来する性差や加齢性変化が関与しているという仮説を立てている。本研究では、in vitroでの角膜内皮細胞や線維柱帯細胞へのCMV感染実験で、性ホルモンや加齢性変化、それに伴う酸化ストレスなどが、角膜内皮細胞の細胞死や線維柱帯細胞の線維性変化に与える影響を検討し、その仮説を検証する。また、患者の前房水や血液サンプルを蓄積し、これらの検証を行う。培養角膜内皮細胞、線維柱帯細胞を用いたin vitro実験については、CMVの感染実験の系の確立に向けて検討を行っている。臨床サンプルの収集については、CMV前眼部感染症患者の前房水サンプルを3例、比較対象となる他のウイルス性虹彩炎の前房水サンプルとして、単純疱疹ウイルス虹彩炎1例、水痘帯状疱疹ウイルス虹彩炎3例、また網羅的感染症PCR陰性症例を10例収集した。

According to reports from home and abroad, CMV infection in the anterior eye of the hospital is 2-4.1 times more common in middle-aged men than in women. The cause of CMV anterior ocular infection is due to sex differences and sex changes. This study examined the effects of CMV infection on corneal endothelial cells and linear pillar cells in vitro, including cell death and linear pillar cell transformation. The patient's anterior aqueous humor and blood are accumulated, and the patient's symptoms are detected. Cultured corneal endothelial cells, linear columnar cells, and the establishment of CMV infection system were studied in vitro. Clinical data collected included 3 cases of anterior aqueous humor loss from CMV anterior ocular infection, 3 cases of anterior aqueous humor loss from other ocular iritis, 1 case of pure herpes iritis, 3 cases of varicella iritis, and 10 cases of PCR negative infection from other ocular infections.
