本研究は、口腔癌がどのように初期発生し、浸潤・転移をきたしていく中でどのようにクローン進化を獲得しているのかを明らかにするため、従来行われてきた“多数の口腔癌症例によるわずかな一点のゲノムシークエンス結果の比較”ではなく、“同一検体内におけるマルチリージョンでの変異解析”を行うものである。また組織内での腫瘍進展ベクトルや本来臨床的には実行困難な腫瘍組織内での時間的経過を遡って推測する、すなわち“口腔癌発生・進展の時間的・空間的多様性”も明らかにする。より効果的なマルチリージョンシークエンスを行うためには、局所進行癌かつ遠隔転移を有する病変の選択が必要不可欠である。病変の切り出し、FFPEサンプル作製、マクロ写真撮影には一定の時間的・人的労力を要する。また多領域のNGS解析に伴うランニングコストも見込んでいる。上記観点から数十検体におよぶ解析は困難と考えている。年次ごとの研究計画として、1年次は検体処置、マルチリージョンシークエンス、結果解析の流れを確立するために、1～2検体の解析完了を目標としていた。まず本研究課題の採択を受けて、当施設の臨床研究審査委員会へ本研究計画の申請を行い、本研究課題採択の期間における臨床検体採取・使用について承認を得ることができた。また病理診断科医師と研究計画について（検体採取法、採取後の検体処理ならびにFFPEサンプル切り出しのタイミングなど）のリサーチミーティングも行った。研究計画に関わる新規アイデアについては各種学会学術大会へ参加し、最新の知見を学ぶことができた。また適当な研究検体の選定ならびに次世代シーケンス解析については、年次内に1解析を選択・委託することができた。当初の計画に合致した進捗調教であり、おおむね順調に進展していると考えている。次年度は検体選定・解析の集積について更にスピード感を持った対応を行いたい。
