唇顎口蓋裂は、日本において比較的高頻度にみられる先天性疾患の一つである。近年では、医療の進歩に伴い、唇顎口蓋裂患者における治療法も発展し、健常児と変わらない機能と形態の獲得が求められるようになってきた。我々は、現在多くの唇顎口蓋裂児に対し、口蓋床を用いた術前顎矯正後、口唇形成術と同時に歯肉歯槽骨膜形成術 (gingivoperiosteoplasty (GPP)) を施行している。本研究では、GPP施行前の術前顎矯正の治療プロトコールの確立を目的とした。そこで本申請では、①術前顎矯正の治療結果に関連する因子の検討と、②口腔内スキャナーによる乳児、新生児の光学印象法の確立を目的として研究を行うことを目的とした。研究計画として、まずはじめに術前顎矯正の治療結果に関連する因子を解析するために、模型の三次元モデル上での各計測項目の測定、関連する因子の探索を行うことと、術前顎矯正の開始時と終了時の上顎歯列形態の変化を観察することとした。大学病院矯正歯科を受診し、出生直後より口唇形成術直前まで術前顎矯正を行ったUCLP児の術前顎矯正開始時、終了時の上顎歯列模型を用い、口腔内スキャナーで術前顎矯正の治療前後歯列模型を撮像し、得られたSTLデータ上に、三次元分析ソフトを用い、三次元モデル上に計測点、基準平面を設定し、距離・角度計測を行った。その結果、8例で両側歯槽突起が接触し(接触群)、5例で接触がなかった (非接触群)。術前顎矯正開始前の鼻中隔下端と正中矢状平面間の水平距離や鼻中隔下端の側方偏位量は、非接触群に比べ接触群で有意差を認め、術前顎矯正開始前の鼻中隔下端と正中矢状平面間の水平距離と、術前顎矯正前後の顎裂間距離の減少量間に正の相関があった。本結果を、日本口蓋裂学会、日本矯正歯科学会にて発表した。学会にて発表後、海外誌 (Cleft Palate Craniofac J)へ投稿し、受理された。

We now have a lot of cleft lip and palate surgery, oral surgery, and gingival periostey (GPP). This study aims to establish the treatment of preoperative jaw correction before GPP. The present application aims to: (1) investigate the factors related to the treatment results of preoperative jaw correction;(2) establish the optical impression method of breast and newborn in the oral cavity; and (3) conduct research. The research plan is to analyze the correlation factors between the treatment results of preoperative jaw correction and the measurement of the correlation factors in the three-dimensional model. There was a positive correlation between the horizontal distance between the lower nasal septum and the median sagittal plane before and after preoperative correction, the lateral deviation of the lower nasal septum, the intentional difference between the non-contact group and the contact group, the horizontal distance between the lower nasal septum and the median sagittal plane before and after preoperative correction, and the decrease in the distance between the cleft palate before and after preoperative correction.
