高齢化に伴い、へき地では整形外科疾患の需要が急増している。一方、近年の新型コロナウイルス感染症の広がりに関連し、県境をまたぐ移動制限や以前から続く医師不足の影響もありへき地での整形外科専門医不在にも拍車がかかっている。そのため、へき地で働く内科医や総合診療医が、整形外科疾患の初期対応を担うことが増えており、整形外科関連の対応能力はますます求められている。また、近年の通信技術の発展や新型コロナウイルス感染症の感染拡大により、遠隔医療の取り組みが世界中で加速しており、遠隔教育への期待も大きい。しかしながら整形外科疾患への教育は手技や臨床判断を含むため、遠隔教育の活用は限定的であった。今回へき地医療を長年経験した総合診療医・整形外科医である研究代表者により、へき地において単独で勤務する総合診療医に対し遠隔整形外科診療教育を行い、遠隔整形外科診療教育の効果を確認する目的で自信度の変化を測定した。またへき地無床診療所における整形外科診療の実数を測定することで総合診療医がへき地で行う整形外科診療の需要を確認した。整形外科分野に関しての遠隔教育に関しては、手術支援に関するものが多く、地域・へき地の外来診療や救急医療を担う医師に対する報告はこれまでに存在しない。今後行う予定の、総合診療医に対し整形外科専門医が遠隔整形外科診療教育を行った報告は初の報告となる予定である。へき地医療機関で働く若手総合診療医に対して遠隔整形外科診療教育を実施したことにより、総合診療医への教育効果を確認することや遠隔整形外科診療教育の運用に必要な因子を明らかにでき、へき地の整形外科関連疾患に対する医療の質を向上させることが可能である。

The purpose of this study is to determine the degree of self-confidence of the research representative of the general medical doctor and plastic surgeon in the field of long-term medical treatment, and to confirm the effect of long-term plastic surgery education. To determine the number of plastic surgeries performed in a bed-free clinic and to confirm the need for plastic surgery performed by a general practitioner. Orthopaedic surgery is a field related to distance education, surgical support, and reporting to outside doctors and emergency medical professionals.
