项目摘要

看護管理者は、看護サービス管理の中核を担う存在であり、わが国では大学院での看護管理学教育や認定看護管理者教育が行われている。本研究は、専門的教育を受けた看護管理者の配置と患者アウトカムとの関連についてリアルワールドデータを用いて明らかにする。本年度はまず第１段階として、日本看護協会が認定している認定看護管理者に着目し、公開されているデータを、地理情報システム等を使用し認定看護管理者の全国分布と所属の有無に関連する地域（二次医療圏）・施設特性を明らかにした。その結果、地域特性では、大都市型や地方都市型の二次医療圏または二次医療圏内に認定看護管理者教育機関が所在している地域により多くの認定看護管理者が存在していることが明らかになった。また施設特性では、病床数が多いこと、認定看護管理者教育機関との距離が近いこと、設置主体が医療法人や個人病院でないこと、DPC算定医療施設であることが、認定看護管理者所属の有無と関連していた。これらから、専門的教育を受けた看護管理者（認定看護管理者）が地域・施設で偏在している可能性が示唆された。さらに、専門的教育を受けた看護管理者が所属する施設および部署概要（施設特性、部署特性、組織環境要因、入院患者特性、看護ケア関連指標）の実態と関連を明らかにするための調査を実施している。調査は2023年3月より開始しており、現時点で十数施設の研究協力の同意を得ている。データ収集は、リアルワールドデータに近いデータを収集するため、各施設の病棟管理日誌やカルテデータより対象者が調査票に転記する形式で収集している。2023年度は、上記調査のデータについて、専門的教育を受けた看護管理者の配置と患者アウトカムとの関連等を検証する予定である。
