项目摘要

本研究は外科手術患者を対象とし、手術直後より実施可能なエクササイズプログラム開発を目的としている。全身麻酔下で行われる手術の多くが術後1日目の離床を目指すが、術後合併症の発生によりそれが叶わないこともある。また、離床ができたとしても安静臥床による弊害で、末梢循環不全の腸管運動障害や筋力低下などの合併用を引き起こすこともある。将来的なエンドポイントはそれらの合併症を予防することであり、今年度は対象者の生体情報や主観的情報により、エクササイズプログラムを完成させる段階にある。末梢循環改善、深部静脈血栓症予防、腸管蠕動運動障害予防、サルコペニア予防について、文献検討および専門家の意見聴取によりプログラムを作成した。手指の掌握運動、足関節の足背・屈曲運動、下肢体幹運動からなる本プログラムについて、健常高齢者を対象としたパイロットスタディを実施し、効果および有害性の検証を行った。その結果、末梢循環の改善が期待でき、呼吸状態・循環動態への負荷は一時的かつ軽度であることを確認した。この検証により、術後患者を対象とした臨床研究へ発展可能であることを確認した。現在、外科手術患者を対象とした検討に向けて、所属大学及び実施施設の研究倫理審査委員会より承認を受け、還流指標やバイタルサインの変化などの客観的データや、インタビュー調査（半構造化面接）で得られた主観的データより、本プログラムの効果を検証するとともに、実施内容を洗練する段階にあり、データ収集を開始するところである。この結果を検討し、対照群を置いた量的研究へと発展させていく予定である。
