＜実施概要＞小児慢性疾患患者における成人移行期支援の長期的効果と移行プロセスの関連要因の解明に向けて、令和4年度は成人移行期支援の類型を明確化するための文献レビュー、および、診療録調査による成人移行期支援の長期的効果の検証を行った。また、研究①【成人移行期支援の実態についての全国調査】の実施へ向けた、質問紙の検討、倫理審査等の準備を進めている。＜実施の詳細＞1)文献レビュー：2022年8月～10月にかけて文献レビュー（スコーピングレビュー）を行った。文献の検索は、MEDLINE, CINAHL, and PsycINFの3つのデータベースを用いて行った。国内外で実施されている成人移行期支援の内容、提供形式、提供者、対象者、効果及び評価指標等についての情報を抽出し、支援内容の類型化を行った。2)診療録調査2022年12月より、都内大学病院1施設において、成人移行期支援の長期的効果を明らかにするための診療録調査を行っている。対象者は、期間内に当該病院の小児科内成人移行期支援外来を受診した患者とし、年齢等の属性、疾患・治療内容・内服や在宅医療的ケア等の疾患に関するデータ、成人移行期支援外来の受診回数、日本語版TRANSITION-Q得点、成人診療科への移行の有無と移行時期、等のデータを収集している。データ収集は、2023年5月をめどに終了する予定であり、R5年度中に分析、執筆、成果報告を進める予定である。
