本研究は、入院高齢心不全患者におけるサルコペニアによる摂食嚥下障害に着目し、以下の4点を明らかにすることを目的に実施する。①入院高齢心不全患者におけるサルコペニアによる摂食嚥下障害の有病率、②入院高齢心不全患者におけるサルコペニアによる摂食嚥下障害と身体機能および低栄養との関連、③入院高齢心不全患者におけるサルコペニアによる摂食嚥下障害の合併が予後に与える影響、④入院高齢心不全患者における肺炎発症を予測するオトガイ舌筋の筋断面積のカットオフ値。本研究における今年度の目標を、研究実施施設における体制作りを完了すること、および信頼性の高い検査方法で患者の登録を開始することとした。今年度の上半期に倫理委員会の承認を得て、関係各所へ説明会を実施することで、臨床研究の組織作りを行った。また、オトガイ舌筋の筋厚を計測するための超音波診断装置を購入し、健常者での測定を行い、検者間信頼性および検者内信頼性を確認した。これらの結果は、先行研究と比較して大きな違いはなかった。これらの成果から患者登録が可能と判断し、下半期から患者のリクルートを開始した。本研究の対象者数は30名を予定しており、順調に患者の登録がすすんでいる。今年度の研究成果は公表するに値しないが、今後データが蓄積されれば、入院高齢心不全患者の栄養介入の方針を決定付ける有益な知見が得られると考える。引き続き、欠損値が出ないよう関係各所と協力しながら研究を遂行していく。

This study aims to address the issue of food intake in hospitalized patients with high cardiac insufficiency and to address the issue of food intake in the following four areas: (1) Incidence of food impairment in admitted patients with high cardiac insufficiency;(2) Relationship between food impairment and body function;(3) Combination of food impairment in admitted patients with high cardiac insufficiency; 4. Prediction of pneumonia in patients with high heart failure admitted to hospital. The purpose of this study is to complete the system of research and implementation, to examine the high level of information and to initiate patient registration. In the first half of this year, the approval of the Ethics Committee, the implementation of the explanation meeting of each institution, and the organization of clinical research were carried out. The measurement of the thickness of the tendon of the tongue shall be carried out in the purchase, normal operation, and internal reliability of the ultrasonic diagnostic device. The results of this study were compared with those of previous studies. The second half of the trial started with patient registration. The number of participants in this study was determined to be 30. This year's research results will be useful for future policy decisions on hospital admissions and nursing interventions for patients with cardiac insufficiency. The relationship between each institute and the research is carried out in cooperation.
