本研究は、入院医療機関における退院後に向けた食支援で、諸外国で着目されるACP：advance care planning（医療者が、家族や家族などケア担当者との間で、個人の価値観や治療の目的について話し合うプロセス）を取り入れ、患者側・医療者/施設側の双方にとって効果的な支援の在り方を検討・提案することを目指している。具体的には、本研究の前段階として実施した研究活動スタート支援「在宅療養の拡充に向けた退院時の食生活支援の在り方に関する検討（課題番号:19K24263）」の成果から開発した、退院時食支援で今後の食生活に向けた患者と管理栄養士など専門家との話し合い（共有意思決定（shered decision making; SDM））を推進する支援ツール（冊子）を実際に活用を進め、その利用可能性の検証、効果検証へと進める計画である。初年度（令和4年度）は、先行研究で開発した支援ツールを入院時に用いるための「退院後に向けた食支援プロセス（（マニュアル）」を構築し、その利用可能性の検証に向けた研究計画を立案した。この退院後に向けた食支援プロセス（マニュアル）では、現在の管理栄養士の栄養食事指導の実態を踏まえ、入院初期と退院前の2回の指導時を中心にアプローチできるよう検討し、共有意思決定支援の実践への適用を促進するために提唱された「3トークモデル」の3ステップ；協働関係を築き退院後の生活について考える必要性があることを共有する「チームトーク」、病院食についての情報提供とともに退院後も病院食をモデルとした食生活を継続することのメリットとデメリットを整理する「オプショントーク」、患者側の価値観やニーズを踏まえて目標設定につなげる「ディシジョントーク」を展開できるようにした。

This study is aimed at exploring the implications of ACP (advance care planning) for post-admission support in hospitals, families, and families, as well as for patient/provider support. Specifically, the research activities carried out in the early stages of this study support the results of the "in-home nursing care and food support at the time of discharge"(Project No. 19K24263), and the results of the "in-home nursing care and food support at the time of discharge" support the development of in-home nursing care and food support at the time of discharge. The relationship between the hospital and the post-hospital life is necessary to provide information on the hospital food after the hospital is discharged. The patient's side of the table is set to the target setting.
