1999年3月に、大阪府下33市の福祉事務所高齢福祉課に所属するホームヘルプ事業の担当者に対して質問紙調査を行ったところ、22市より回答を得た。さらに、回答自治体のうち6つには、追加的な聞き取り調査を行った。以下、調査結果から発見されたことを、6点に要約する。(1)特養ホームの利用率に関する自治体間格差よりも、ホームヘルプの利用率に関する格差の方が大きい。(2)在宅福祉サービスの整備状況が低位の自治体では、特養利用者数とヘルパー利用者数のバランスが他の自治体と異なる。(3)老人保健福祉計画の実施段階において、多くの自治体は、恒常的に予算の未消化を経験していた。(4)ホームヘルプ事業の担当者の大部分は、計画の期間内達成を困難と感じている。その理由としては、「目標事業量が高すぎた」、「市民のニーズがない」と答える者が多い。(5)回答自治体の約半数では、1999年度の予算編成過程において、担当者が起案した予算要求を財政課に減額査定されている。しかしながら、結果として決定された予算額について、担当者の多くは「市民のニーズから見れば妥当な額」と考えている。(6)ホームヘルプ事業の整備状祝に関して自治体間格差が拡大しつつある原因のひとつとして、市福祉事務所に勤務する担当者の裁量が考えられる。とくに、事業予算の起案、および実績を拡大するためのさまざまな活動をめくる裁量は、非常に大きいようである。なお、このほかの要因としては、この事業をめぐるいわゆる「事業体依存」なども考えられる。
