项目摘要

平成10年度に超音波心筋組織診断が可能なように市販の超音波心エコー装置に改良を加え,アドリアマイシンを含むanthracycline系抗がん剤投与患者の心筋組織性状診断をIBを用いて行い,anthracycline系抗がん剤投与患者においては従来の心エコーで評価した心機能指標が正常にも関わらずIB指標が異常である症例がみられることを第63回日本循環器学会学術集会(1999年3月27〜29日,東京)で発表した.平成11年度は比較的単一のプロトコールで治療されている非ホジキンリンパ腫患者を対象に心筋組織性状診断を行い,アドリアマイシン投与量とIB指標,従来の心エコー指標を比較した.未治療群(C群)とアドリアマイシン治療群(L群,DXR総投与量≦200mg/m^2;M群,≦400mg/m^2;H群>400mg/m^2)において各群間で従来の心エコー指標には差がみられなかったが,IBの心周期変動(CV-IB)はアドリアマイシン総投与量の増加により低下し,C群,L群に比しH群で有意に低値であった.以上よりIBの解析により早期にアドリアマイシンによる心筋障害が検出できる可能性が示唆された.上記の要旨は第15回北陸心筋代謝研究会(2000年2月26日,金沢)において発表した.現在論文化の準備を進めている.
