ヒトパピローマウィルス(Human papilomavirus:HPV)は種々の疣贅の原因ウィルスとして古くから知られている。そのなかで、表皮内癌の1つであるBowen病からHPVの検出が相次いで報告され、HPV感染が表皮細胞の癌化への関与する可能性が示唆されている。我々は、Bowen病の前駆段階としてdysplastic wartsという皮膚疾患を新しく提唱した。この皮膚疾患は、臨床的にはウィルス疣贅様の腫瘤を形成し、組織学的にウィルス性疣贅に特徴的な表皮細胞の空胞変性を認めながら、細胞の異型がある。さらに我々のDNA解析による検討では、この皮膚疾患からHPVをSouthern blot hydridization法により検出しており、表皮細胞の癌化へのHPVが関与している可能性が示唆される疾患であると考えられた。そこで今回、表皮の癌化におけるHPVの役割の詳細を明らかにするために、東京女子医大皮膚科外来において生検時にdysplastic wartsの9例について、病理組織診断にあわせてDNA解析についても行った。DNA解析はHPVの存在をSouthern blot hybridization法とPCR法で調べ、さらに陽性例についてはHPVの遺伝子型をRFLP法とdirect sequence法により同定した。その結果、子宮頸部癌において頻度が高く検出されていると考えられているHPVの遺伝子型が検出された。in situ hybridization 法により組織学的に病変内のHPV DNAの局在が明らかとなり、この皮膚疾患におけるHPVの役割が明らかにされた。症例数が少ないので、今後さらに症例数を増やして解析する必要があるとともに、HPVと表皮の癌化について機能的な面も含めた解析が必要である。

Less disease cases が な い の で, future さ ら に disease cases を raised や し て parsing す る necessary が あ る と と も に, HPV の と skin cancer に つ い て function of な も containing め た parsing が necessary で あ る.
