アトピー性皮膚炎(AD)発症の基盤となるatopic skinにおけるバリアー機能障害は、角質細胞間脂質とくCこセラミドの減少が関与していることが明らかになっている。このセラミド量の減少は末だADと診断し得ない乳児湿疹患者において、家族にアトピー素因を有する群では家族歴のない群に比し、より顕著であることから、遺伝的素因に基づくことが示唆される。セラミド量減少の成因は、角質を用いた検討でスフィンゴミエリン水解活性が著しく亢進し、アシラーゼ活性をもつ別の酵素が誘導されるためと考えられている。そこで、このスフンゴミエリン代謝異常が遺伝的に規定された異常、すなわち先天的な代謝異常であるか否かを、リンパ球を用いて検討した。AD患者20例の前腕屈側の皮疹部よりテープストリッピンクにて採取した角層と、更に同部位よりパンチ生検にて採取した表皮と、同一患者の末梢血リンパ球を材料とし、今回新たに脂肪酸標識したスフィンゴミエリンを基質として用い、アシラーゼ活性を測定した。その結果、角層および表皮におけるアシラーゼ活性は正常人に比べ著しく亢進していたが、リンパ球においては有意な変動は認められなかった。このことは、セラミドの代謝異常が全身的な代謝異常の一部分症ではなく、AD患者皮膚に特異的に限局した代謝異常であることが示唆された。
