本研究は全部床義歯装着者における咀嚼能力と顎堤粘膜の疼痛閾値との関係を明らかにするために,1)顎堤粘膜の荷重測定装置を開発し,ついで,2)全部床義歯装着者の粘膜の疼痛閾値を測定し,3)咀嚼能力と疼痛閾値の相関について,解析することを目標とした.前年度に,ストレンゲージを組み込んだ疼痛閾値測定システムを製作し,口腔内での動作確認は終了している.さらに較正実験により口腔内使用における動作の安定性ならびに測定値の精度再現性は良好であり,実際の測定に十分であることも判明している.本年度はまず,有歯顎者を対象とした口腔粘膜測定の再現性を確認した.有歯顎者を対象とし疼痛閾値測定装置を用いて口蓋粘膜,下顎頬側,舌側の粘膜の疼痛閾値の繰り返し測定を行い,得られた測定値について被験者,被検者内の日内変動および日間変動した.その結果,変動はそれぞれ,すべての測定部位において約100g,および約150gであった.さらに,義歯装着者の疼痛閾値の測定ならびに咀嚼能力の測定を行った.義歯に満足している全部床義歯装着者において,上顎;中切歯,犬歯,小臼歯,大臼歯部それぞれの顎堤頂,口蓋皺壁部口蓋中央部,口蓋小窩,下顎;中切歯,犬歯,小臼歯,大臼歯部それぞれの顎提頂,顎舌骨筋線部,小臼歯舌側,大臼歯頬側を測定ポイントとし,疼痛閾値を測定した.その結果,上顎口蓋側と下顎舌側においては頬側部の約1/2の数値を示した.また,内田による食品アンケート調査用紙を用いた咀嚼能力は平均65点を示した.咀嚼能力と下顎頬側部の顎堤粘膜の疼痛閾値の相関は弱い相関を示した.

The purpose of this study was to clarify the relationship between masticatory ability and pain threshold of jaw mucosa in all patients with dental prostheses, 1) develop a device for measuring the load of jaw mucosa, 2) measure the pain threshold of jaw mucosa in all patients with dental prostheses, 3) analyze the relationship between masticatory ability and pain threshold. In the previous year, the pain threshold was determined and the oral movement was confirmed. The stability of the operation and the accuracy of the measurement are good, and the measurement is very accurate. This year, the reproducibility of oral mucosa measurement was confirmed by the examination of oral mucosa. The pain threshold value of oral mucosa, buccal mucosa and lingual mucosa was measured by using the pain threshold value measuring device. As a result, the measured position of the sample was about 100g, about 150g. In addition, the measurement of pain threshold and masticatory ability of dental prostheses wearers was also carried out. The pain threshold was measured at mid-range, upper jaw; mid-range, canine, small acetabular, and large acetabular teeth; mid-range, canine, small acetabular, and large acetabular teeth; mid-range, canine, small acetabular, and large acetabular teeth. As a result, the upper jaw flap side and the lower jaw tongue side are about 1/2 of the value of the cheek side. The average chewing ability of Uchida's food was 65 points. The correlation between masticatory ability and pain threshold of buccal mucosa was weak.
