当科で加療した口腔扁平上皮癌73症例の臨床所見ならびに病理組織所見と頚部リンパ節転移について検討した。頚部リンパ節転移を認めた症例(原発巣再発に伴う頚部リンパ節転移をのぞく、以下転移群)は27/73例(37.0%)にみられ、T分類(UICC;1997),分化度、癌浸潤様式別に転移群の割合を算出した。【T分類】T1;5/10(50%),T2;11/35(31%),T3;1/6(17%), T4;10/22(38%)【分化度】高分化型;9/35(26%),中等度分化型;15/33(45%),低分化型;3/5(60%)【癌浸潤様式】2型;2/20(10%), 3型;12/34(35%), 4C型;8/13(62%),4D型;5/6(83%).【免疫組織学的所見】P53、MIB-1蛋白の発現を検索した。各々の因子において、その発現率から発現しないもの、5%未満、5%以上、10%以上、50%以上と大別して頸部リンパ節転移率を比較した。P53蛋白の発現と頸部リンパ節転移の有無との間に明らかな関連性はみられなかったが(4/10、6/15、4/11、7/17、6/18)MIB-1蛋白の発現率が10%以上の症例群では転移症例の頻度が高い傾向であった。(0/0、7/17、5/19、15/33、0/4)これら治療前の臨床所見、組織学的所見を独立変数に、頚部リンパ節転移の有無を従属変数として多変量解析(ロジスティック回帰分析)した結果、癌浸潤様式(P=0.179)のみ統計学的に有意な転移関連因子と考えられた。癌浸潤様式は腫瘍と宿主の相互作用の結果を形態的に評価したものであり、分子生物学的な因子の産物である各種蛋白の発現に比べて臨床的に有用であった。この結果をふまえ、浸潤様式別に分子生物学的な観点から解析を加えることでさらに確度の高い転移関連因子を検索する必要性が考えられた。

Clinical findings and pathological findings of 73 cases of oral squamous cell carcinoma were studied. 27/73 cases (37.0%) were classified as T type (UICC;1997), and the differentiation degree and invasion pattern were calculated. T1; 5/10(50%),T2;11/35(31%),T3;1/6(17%), T4;10/22(38%)[differentiation] well-differentiated type;9/35(26%), moderately differentiated type;15/33(45%), poorly differentiated type; 35 (60%)[invasive pattern] type 2;2/20(10%), type 3;12/34(35%), type 4C;8/13(62%), type 4D; 5/6(83%). [Immunohistological findings] P53, MIB-1 protein expression was detected. (0/0, 7/17, 5/19, 15/33, 0/4) Clinical findings before treatment, histological findings, presence or absence of node shift, and statistical correlation factors (P=0.179). The results of tumor invasion and host-host interaction are morphological, molecular and biological factors that are useful in the development of various proteins. The results of this study are based on the analysis of molecular biology and the necessity of determining the correlation factors of high degree of accuracy.
