【目的】深部静脈血栓症(DVT)の診断は、身体所見、下肢静脈エコー、凝血学的検査等を用いて行われる。凝血学的検査のうち、フィブリン分解産物(Dダイマー)はその診断的有用性が確立されているが、それ以外の検査マーカーの有用性は明らかではない。本研究は、DVTが高率に発症する可能性がある震災時において、被災者に対し下肢静脈エコーを施行し、凝血学的マーカー(フィブリン/フィブリノゲン分解産物 : FDP、Dダイマー、可溶性フィブリン : SF、プロトロンビンフラグメント : F1+2、トロンビンーアンチトロンビン複合体 : TAT、プラスミン・α2プラスミンインヒビター複合体 : PIC)測定を行い、DVTの頻度およびDVTとマーカーの関連性について検討した。【方法】能登半島地震発生後に避難所生活を余儀なくされていた一般住民でインフォームドコンセントが得られた237例のうち、下肢静脈エコーと凝血学的検査を行い得た197例(平均年齢 : 72.4±11.5歳, 女性 : 133例)を対象とした。1. 問診の結果、2. DVTの頻度、3. DVTと凝血学的マーカーの関連性について解析を行った。問診、DVTの頻度、DVTとFDP、Dタイマー、SFにおける関連性の結果の一部は、前年度の科学研究費補助金実績報告書にて報告した。【結果】(1)DVT陽性群(n=28)のF1+2 (平均値400±193pmol/mL)、TAT (3.7±4.6ng/mL)、PIC (1.4±0.9μg/mL)は、陰性群(n=169)のF1+2 (306±155pmol/mL)、TAT (2.1±3.9ng/mL)、PIC (1.0±0.4μg/mL)に比しいずれも有意に高値であった(p<0.01、FDPおよびDタイマーはp<0.001で有意差あり)。陰性群の中にSF同様、複数マーカーにて高値を示す症例が存在し、血栓形成準備状態が示唆された。(2)ROC曲線から算出したF1+2、TAT、PICの曲線下面積は、0.658、0.694、0.657であり、FDP (0.765)およびDタイマー(0.762)においてDVTの予測能・診断能が高いと考えられた。(3)感度と特異度は、F1+2 (78.6、49.7)、TAT (78.6、55.0)、PIC (53.6、75.1)であり、FDP (85.2、65.9)およびDタイマー(85.2、64.7)が高値であった。SFは、両群で有意差無し(p=0.102)、曲線下面積は0.595、感度および特異度(78.6、42.6)と最も低かった。【まとめ】今回の検討では、凝血学的マーカーのうちFDPおよびDタイマーがDVT検出に有用であると考えられた。SFに関しては、DVT発症1日目に上昇するため検査時期が影響した可能性があり、更なる検討が必要である。また、他のマーカー(F1+2、TAT、PIC)を含め震災時のみならず、悪性腫瘍や術後等の疾患を追加し、急性期発症DVTに有用なマーカーを検討していきたいと考える。
