深部静脈血栓症(DVT)の診断は、身体所見、下肢静脈エコー、凝血学的検査などを用いて行われる。凝血学的検査のうち、フィブリン分解産物(Dダイマー)はその診断的有用性が確立されているが、それ以外の検査マーカーの有用性は明らかではない。本研究は、DVTが高率に発症する可能性がある震災時において、被災者に対し下肢静脈エコーと凝血学的マーカー(フィブリン/フィブリノゲン分解産物 : FDP、Dダイマー、可溶性フィブリン : SF)測定を行い、DVTの頻度およびDVTとマーカーの関連性について検討した。能登半島地震発生後に避難所生活を余儀なくされていた一般住民で、インフォームドコンセントが得られた237例(平均年齢 : 72.2±11.5歳、女性 : 157例)を対象とした。1. 問診の結果 : 地震発生6日および9日目では16.5%の症例に、100日目では34.8%の症例に下肢の浮腫・痛み・腫れを認めた。2. DVTの頻度 : 下肢静脈エコーを行った237症例中28例(11.8%)にDVTを検出した。3. DVTと凝血学的マーカーの関連性(1)DVT陽性群(n=27)のFDP(平均値6.5±1.97μg/mL)およびDダイマー(3.3±1.00μg/mL)は、陰性群(n=170)のFDP(1.9±0.10μg/mL)およびDダイマー(0.93±0.05μg/mL)に比し共に有意に高値であった(p<0.001)。一方、SFは両群で有意差を認めなかった(p=0.102)。陰性群の中にSF高値を示す症例が存在し、血栓形成準備状態が示唆された。(2)ROC曲線から算出したFDP、Dダイマー、SFの曲線下面積は、0.768、0.775、0.598であり、FDPおよびDダイマーにおいてDVTの診断能が高いと考えられた。(3)感度と特異度は、FDP(85.2、65.9)およびDダイマー(85.2、64.7)が、SF(77.9、41.8)に比べて高値であった。今回の検討では、凝血学的マーカーのうちFDPおよびDダイマーがDVT検出に有用であると考えられた。SFに関しては、今後更なる検討が必要である。また、他のマーカー(プロトロンビンフラグメント : F1+2、トロンビン-アンチトロンビン複合体 : TAT)を追加測定、更に疾患別による症例数を増やし検討予定である。
